Model Contract

If you decide that you want to participate in the Modeling Program, you will be asked to sign a modeling contract at the time of your consultation.  Below is an example of what the contract will read.

I, ________________________________, agree to model for Gia Wood, for any and all photographic material she acquires from the Photo Shoot on  _____________.  This photo shoot is for my participation in the Modeling Program.  I understand that the services listed below are at a discounted rate based on my participation in this program.  Should I receive any of the services below at the discounted rate, and not take pictures for Gia Wood within seven (7) calendar days of those services, I will be held financially responsible for the full price of each service I received, and a non refundable $300 cancellation fee. I am fully aware that these photos will be used on www.giawood.com, for promotional purposes including brochures, business cars, and flyers, and any other type of media Gia Wood would like to use them in so long as it is in association with Gia Wood DBA Hair by Gia.
